This year, due to the COVID-19 pandemic, we will be doing things a little different to register your child. Normally, you would visit the school and complete a ton of forms for registration. To make things a little easier, we have created the registration surveys below, which are questions from all of the enrollment forms combined

PLEASE BE ADVISED, IN ADDITION TO SUBMITTING THIS SURVEY, YOU MUST EMAIL THE REQUIRED DOCUMENTATION:
-Birth Certificate or Passport -Immunization Records -2 Proofs of Address
Please take pictures of your required documentation (if you are not able to scan), email them to 19K557@schools.nyc.gov and state your child's name in the subject line.
