How to Complete the DOE Health Screening Form

The following is an overview of how to complete the DOE's health screening form.

Go to https://healthscreening.schools.nyc/ and select "Guest Screening." If you need to select a different language, use the drop-down box at the top of the page.
2. On the next page, fill in your student's information, and then the parent or guardian's email. For "Facility," search for our school. Search for "Brooklyn Gardens Elementary School (K557)" for the best result. Then click "Fill out daily screening."
3. On the next page, the questions appear one at a time. Fill in each question, then click "Submit screening." For this example, "no" was selected for everything. When you fill out this form, please do so accurately.
4. Based on your responses, you will receive the appropriate screening form. Please note the form must be filled out daily.
That's it! Show this pass when you arrive at school, either on your phone, or printed out.
